Applications must include a CV and cover letter, as well as a completed Midland Anaesthetic Training Program checklist.  [ The checklist document is available from the DHB vacancies website once applications open.  It simply offers a reminder to ensure that your application is complete, and requests simple information from you to streamline the process for the recruitment team.  It normally takes candidates less than a minute to complete! ]  We require references to be submitted at the time of application; applications without completed references are severely disadvantaged.  References are required on the MCNZ RP6 form.  Referees should email their reference directly to This email address is being protected from spambots.
